Aging As The Central Risk

Aging sits beneath many of the conditions we name separately. Cancer, heart disease, Alzheimer's disease, diabetes, and cognitive decline do not arrive from one source, yet the passage of decades changes the terrain in which they take root. Longo frames this directly: smoking and obesity matter, but 30 years of aging can dwarf them as a driver of major disease risk.

That comparison does not minimize personal choice. It sharpens it. If aging is the larger force, then longevity work cannot be reduced to avoiding obvious harms. It must ask how the body decides between growth and protection, between building and repair, between constant abundance and deliberate pause.

Longo came to this question through calorie restriction, a field shaped by decades of animal research and by mentors who treated food intake as a serious lever in aging biology. Chronic restriction showed power, but it also revealed its burden. A protocol that demands daily restraint for years asks too much of most people, and in older organisms it can raise concerns around weakness, immune function, and lean mass.

The more intentional question became: can a short nutritional ritual create a protective state without requiring a life of continuous restriction. In the source, this is the pivot from calorie restriction toward periodic intervention. Five days, repeated at intervals, becomes a way to invite the fasting response, then return to nourishment.

Nutrition matters here because it is practical, scalable, and already connected to the core pathways that regulate growth, repair, and survival. Exercise belongs beside it, but food sends continuous signals. Protein, amino acids, sugars, fats, and total calories speak to the body every day. The body listens, then chooses where to invest.

In abundance, biology supports growth and reproduction. Under structured scarcity, it shifts toward maintenance and defense. That tradeoff is the foundation of the fasting mimicking diet: not deprivation for its own sake, but a deliberate reset that asks the body to conserve, repair, and restore equilibrium.

how do we is there a way to replace these very difficult strategies that are these very difficult strategies that are used to to uh delay aging uh and uh and used to to uh delay aging uh and uh and so that's what I'm going to talk about so that's what I'm going to talk about today this this fasting making diet and today this this fasting making diet and the idea that maybe you can intervene the idea that maybe you can intervene periodically and maybe as little as once periodically and maybe as little as once every three or four months for five days every three or four months for five days and that can have long lasting effects and that can have long lasting effects and some of these effects are are very and some of these effects are are very much uh surprising so these are uh my uh much uh surprising so these are uh my uh disclosure statements and um because of disclosure statements and um because of my interest in aging I always often my interest in aging I always often start with this uh slide and and these start with this uh slide and and these are the risk factor for cancer heart are the risk factor for cancer heart disease and disease and Alzheimer's uh and you see smoking and Alzheimer's uh and you see smoking and obesity keep coming up as the major risk obesity keep coming up as the major risk factors for these diseases and of course factors for these diseases and of course there are major risk factors for all there are major risk factors for all kinds of diseases and kinds of diseases and then uh I was struck by this slide that then uh I was struck by this slide that a friend of mine Matt kimberland gave to a friend of mine Matt kimberland gave to me uh which is is look at what happens

mice live 40% longer they have lless cancer the cancers most of the the cancer the cancers most of the the tumors are actually benign versus tumors are actually benign versus malignant and they they get them later malignant and they they get them later in life right so they get they're in life right so they get they're protected from diabetes they protected protected from diabetes they protected from cognitive decline and uh and so from cognitive decline and uh and so help with these little mice that I help with these little mice that I showed you earlier get to this very long showed you earlier get to this very long lifespan without any diseases versus lifespan without any diseases versus less than 10% for the wild type right so less than 10% for the wild type right so not only they live a lot longer but they not only they live a lot longer but they get there with no diseases um so what get there with no diseases um so what about the people and here you see the um about the people and here you see the um on the left cancer dats they're they on the left cancer dats they're they rarely die of cancer they rarely develop rarely die of cancer they rarely develop diabetes cognitively I'm not showing it diabetes cognitively I'm not showing it but we actually brought them to Los but we actually brought them to Los Angeles did fmis and they are they have Angeles did fmis and they are they have a younger um cognitive function a younger um cognitive function phenotype and maybe similar to those to phenotype and maybe similar to those to that of somebody 20 years younger uh that of somebody 20 years younger uh than they are so those that lack the than they are so those that lack the gron receptor seem to have all these uh gron receptor seem to have all these uh all these effects just like like uh what

all these effects just like like uh what we had seen in in the mice so so if you we had seen in in the mice so so if you look at this network and this is a very look at this network and this is a very much a simplification of the network much a simplification of the network because different cells have different because different cells have different network but there some of the usual susp network but there some of the usual susp suspects are these so grone igf1 Tor suspects are these so grone igf1 Tor ases on one side responding to the ases on one side responding to the protein proteins and specifically to um protein proteins and specifically to um to certain amino acids and on the other to certain amino acids and on the other side the sugars and the insulin side the sugars and the insulin signaling and Ras PKA signaling and so signaling and Ras PKA signaling and so how do you control these um in the calor how do you control these um in the calor restriction field that I was telling you restriction field that I was telling you earlier uh in Roy Walford for a 100 earlier uh in Roy Walford for a 100 years they've been looking at something years they've been looking at something called calorie restriction so what if called calorie restriction so what if you just reduce the all the calories um you just reduce the all the calories um by about 25% right so you reduce the by about 25% right so you reduce the proteins the amino acids the sugars the proteins the amino acids the sugars the fats equally and and in fact there's a fats equally and and in fact there's a lot of data in uh from from mice and lot of data in uh from from mice and rats and that showing that um the this rats and that showing that um the this extend lifespan but this is very

extend lifespan but this is very difficult and also there's data when difficult and also there's data when eventually they did the study the color eventually they did the study the color restriction study monkeys the monkeys restriction study monkeys the monkeys lived a little bit longer but uh lived a little bit longer but uh probably uh were showing the evidence probably uh were showing the evidence for tradeoffs right so they were weaker for tradeoffs right so they were weaker uh they're living longer more frail at uh they're living longer more frail at least from some point of view maybe the least from some point of view maybe the IM the immune system certainly muscle IM the immune system certainly muscle loss Etc so um so is it possible then loss Etc so um so is it possible then that you can intervene uh that you can intervene uh periodically um and and and generate a u periodically um and and and generate a u you know starvation response state for you know starvation response state for just a few days and is it possible that just a few days and is it possible that that will last a long time um and of that will last a long time um and of course they will control all these course they will control all these Pathways but it will control them not Pathways but it will control them not just acutely but long term right so just acutely but long term right so growth ormon gf1 T PKA is it possible growth ormon gf1 T PKA is it possible that the short intervention is causing that the short intervention is causing long-term effects on all these markers

so I'm today I wanted to talk about a recent paper um which by by Amanda in recent paper um which by by Amanda in the lab and in Amanda what we wanted to the lab and in Amanda what we wanted to do for a long time I mean most people do for a long time I mean most people don't want to change your diet right and don't want to change your diet right and and of course we we we're going to keep and of course we we we're going to keep saying change your diet and have much saying change your diet and have much healthier diet but most people are not healthier diet but most people are not going to do that so we always wanted to going to do that so we always wanted to do a study about what happens what if do a study about what happens what if you just allow somebody to have the you just allow somebody to have the whatever diet they had before and you whatever diet they had before and you just intervene once a month for 5 days just intervene once a month for 5 days of course uh this is a mouse study of course uh this is a mouse study because in M we get to do a lifespan and because in M we get to do a lifespan and we get to look at everything like so a we get to look at everything like so a lifelong study of a high calorie Western lifelong study of a high calorie Western diet high sugar high fat Western diet um diet high sugar high fat Western diet um versus the versus the same diet but with five days a month same diet but with five days a month fasting mimicking diet so 25 days of fasting mimicking diet so 25 days of this bad diet five days of the fasting this bad diet five days of the fasting mimicking diet and uh and so
