From Illness To Inquiry

A life in medicine can begin with certainty, then be remade by the body’s refusal to follow a simple script. Dr. Kent Holtorf entered training with the same evidence-based discipline that shapes conventional care. He trusted the system he had been taught to trust. Then Lyme disease, Babesia, Bartonella, and heart failure altered the terms of his life.

The illness was not abstract. Talking to patients became exhausting. Walking upstairs became a test of will. His heart, affected by fibrosis in the setting of infection and inflammation, could not fill as it should. The physician became the patient, and the patient could not function.

That crisis created a new kind of inquiry. Holtorf had been trained to view integrative and alternative medicine as unsupported. Like many physicians, he absorbed a clear hierarchy of evidence and authority. Yet the consultations he sought inside the standard system did not restore his capacity, and the explanations he received did not match the reality of his decline.

I couldn't stand up straight. I couldn't walk upstairs.

When the expected path no longer held, he began looking elsewhere. He traveled for protocols that were not yet part of ordinary American practice, testing ideas with the urgency of someone who needed function, not theory. Some interventions helped and then faded. Others opened a door, but did not keep it open.

In Europe, he encountered peptides. He did not arrive as a believer. He tried them with limited expectation, then noticed a change within days. Walking upstairs felt different. The body offered a signal that his training had not prepared him to receive, and he followed it with precision.

The improvement did not make him less evidence-based. It made him more demanding about what evidence must answer. Recovery is not an ideology; it is function returning to tissue, breath, movement, and attention. does not it's it it's damaged to even more than the thyroid so they have uh more than the thyroid so they have uh this is the pattern of a low thyroid this is the pattern of a low thyroid chronically ill chronically inflamed chronically ill chronically inflamed person is a low normal TSH a high normal person is a low normal TSH a high normal T4 and I'll tell you why and then a low T4 and I'll tell you why and then a low normal 3t3 and a high normal reverse G3 normal 3t3 and a high normal reverse G3 but if most doctors will look at that but if most doctors will look at that they go oh you got low normal TSH and a they go oh you got low normal TSH and a high normal T4 oh you're a little high normal T4 oh you're a little hyperthyroid it's the opposite and hyperthyroid it's the opposite and because the thyroid doesn't get because the thyroid doesn't get especially T4 does not get into the cell especially T4 does not get into the cell because of its active transport all the because of its active transport all the integr think is just diffuses in T3 is integr think is just diffuses in T3 is less energy dependent than T4 and also less energy dependent than T4 and also the dionas is would convert T4 to T3 in the dionas is would convert T4 to T3 in the body with any the inflammation age the body with any the inflammation age stress it goes down that the DI type one stress it goes down that the DI type one uh is is down regulated so you get less uh is is down regulated so you get less T4 T3 in the pituitary it goes up it's T4 T3 in the pituitary it goes up it's stimulated so you get more conversion so stimulated so you get more conversion so TSH goes down as the rest of the body TSH goes down as the rest of the body starving for thyroid starving for thyroid and that's you look at depressed
